Form 9465 — Installment Agreement Request

We e-file this request with the IRS. Before anything is sent, this page checks it against the IRS's Form 9465 rules and tells you here if something must change.

Free to prepare and e-file. The IRS may charge its own setup fee, paid directly to the IRS.

The IRS does not take these requests electronically — they must be mailed on paper:

  • You owe more than $50,000 on line 9 (paper Form 9465 with Form 433-F).
  • You want to pay by payroll deduction (paper Form 9465 with Form 2159).
This site files the IRS test system's package (CUv29.0, Form 9465 Rev. December 2026). On that form the monthly payment is line 10, the payment day is line 11 and direct debit is lines 12a to 12c; the old line 10 (line 9 ÷ 72) is not filed, and the rules that a payment must reach it and that a $25,000 to $50,000 balance needs direct debit do not apply. Test SSNs only (4th and 5th digits 00).

Fields marked * are required.
